If you have questions about completing any of the forms, please view the Contact page on this website, and e-mail the appropriate Distance Learning Center Representative for the respective area in which you need assistance.
Use this form to apply for graduate admission to the TSU Distance Learning Center. Please note that your application WILL NOT be processed until receipt of the $40 application fee. Payments NOT made by credit card, may be made directly to the Distance Learning Center by check or money order ONLY.
The application for readmission may be completed by prior TSU students who have been out of school for at least one calendar year and who are in good academic standing.
Use this form if it has been less than one year since you submitted an application for admission and have ALREADY PAID the $40 application fee, but you now wish to admit into a different program and/or concentration. There is no admission/application charge for changing programs or concentrations. If you have been out of school for more than one calendar year, then you must use the Readmission Application in the previous section.
After completing the TSU Distance Learning Graduate Application for Admission, use this Secure Credit Card Authorization Form if you wish to pay for the admission fee online with your credit card.
After completing the Application for Admission, complete the Online Advisement Form for your chosen program. You may find the links to the Online Advisement Forms within each program, or you may link directly to them here:

Use this form to register for online courses. The link to this form will ONLY be active when registration is open. All admissions requirements must be met before a student is permitted to register.
This form should be used when you intend to drop one course, withdraw from all courses, or add one course. Please note that you cannot take more than two courses in a term without consultation with the Director of Student Services, Dr. Barbara Echord. Please check the drop/add/withdraw dates on the Term Schedule before completing this form.
After completing the Registration Form, use this Secure Credit Card Authorization Form to pay for the tuition and fees online.
After you have completed the Registration Process, you must immediately complete the Course Exam Proctor Form. The Course Exam Proctor Form provides information about your proctor to the Distance Learning Center. A suggested list of approved proctors is included on the form.
If you are enrolled in a program that requires a final Comprehensive Exam, then complete this form the term before you are ready to take your exam. Please note there is a $50.00 fee to take the Comprehensive Exam.
Please complete the Change of Information Form if you have a change in any of your personal information. It is important to keep the Distance Learning Center informed of any changes in your personal contact information.
Use the Transfer of Location Form if you are a student at another TSU branch, but wish to transfer your student records to the Distance Learning Center.

The Statement of Understanding Form is an Adobe pdf document that must be completed and faxed to the Distance Learning Center if a student has not submitted all official records to the Distance Learning Center by the end of their first term. The student must complete the Statement of Understanding before they may enroll in a second term.
The Petition For and Work To Remove an Incomplete Grade is an Adobe pdf form that should be printed and faxed if you need to 1) request an incomplete grade, and 2) develop a plan of work with your instructor to remove the grade of "I" from your records when all work has been completed.
